Please use your  real name and date of birth when asked for your “Full Name” during registration -  e.g. Full Name: -You can choose to hide this personal information.  Your user name  does not however need to be your real name.  When applying to register we will confirm that these details match your records before authorizing your registration on the forum.  Please note that if we are unsure we may contact you using the details we already hold on your medical record to confirm your identity.
